If you submit a form through a Facebook or Instagram ad, Meta Platforms, Inc. collects that information and shares it with us so we can respond. Meta’s own privacy policy governs how Meta uses information. Please don’t include health details in these forms; our team will talk with you about your dental needs directly.
Your health information
Information about your dental care and treatment is protected health information under HIPAA. We handle it as described in our Notice of Privacy Practices, which is available at any of our offices and on request.
Calls, texts and email
If you ask us to contact you, we may call, text or email you about your request. Message and data rates may apply. You can ask us to stop at any time by replying STOP to a text or by contacting any of our offices.
Your choices
You can ask us to update or delete the contact information you’ve submitted, or to stop contacting you, by reaching out through kyadc.com/contact or calling any of our offices. Most browsers also let you block or delete cookies.
